How to break a recurring problem cycle

The most effective first step a leader can take to break a recurring problem cycle is to stop treating the symptom and commit to diagnosing the root cause.

Recurring problems often persist because the initial solution only addressed the visible effects (symptoms), rather than the underlying reason (root cause). A leader's commitment to a rigorous, non-blaming diagnosis is the essential first move toward permanent change.

Commit to Root Cause Analysis (RCA)

Instead of immediately brainstorming a new quick fix (the "plunging-in-bias"), the leader must initiate a structured investigation to understand why the problem keeps happening.

  • Define the Problem Clearly: The recurring issue needs to be stated in clear, specific, and measurable terms, differentiating facts from opinions.
  • Create a Blame-Free Environment: A leader must set the tone that the goal is not to assign blame to a person but to understand the system or process failure. This transparency and psychological safety encourage team members to share crucial, honest information.
  • Utilise RCA Tools: Standard structured approaches should be used, such as:
  • — The 5 Whys: Repeatedly asking "Why?" (typically five times) to peel back layers of symptoms until the fundamental cause is reached.
  • — Fishbone (Ishikawa) Diagram: A visual tool that categorises potential causes (i.e., Man, Method, Machine, Material, Measurement, Environment) to help a team see all the contributing factors.

Subsequent Key Steps for Leaders

Once the root cause is identified, the leader's role shifts to implementing and sustaining the long-term solution.

Implement a Systemic Solution

Focus the solution on the identified root cause, not the symptom. This often involves changes to training, procedures, policies, or technology, rather than simply telling people to "do better."

Ensure Follow-Up and Accountability

  • Implement Corrective Action: Put the new solution into practice.
  • Measure and Monitor: Establish metrics (KPIs) to track whether the new solution actually solved the problem and is preventing recurrence.
  • Reinforce: Consistently check in and ensure the new process or system is being followed, preventing a slow drift back to the old, problematic way of working.

Why #: 4th Why | Question: Why did the engineers feel they didn't have enough time? | Answer/Finding: The project manager was continually adding scope changes during the development phase to meet client requests.

Why #: 5th Why | Question: Why was the scope continually being changed and added mid-development? | Answer/Finding: The initial project scoping meeting excluded the lead engineer, resulting in an underestimation of technical complexity and an inadequate process for controlling scope creep.

The recurring problem of missed deadlines is not due to "slow engineers" or "bad testing," but the lack of a formalised scope control and approval process, that involves technical leads from the start.

From Root Cause to Corrective Action

As the leader, the most effective corrective action is now clear. Instead of simply telling the team to "test harder" (treating the symptom), you must implement a system change to address the root cause:

  • New Policy: Mandate that all initial project scoping meetings must include the relevant technical lead (or a designated senior representative) to provide accurate estimations.
  • New Process: Implement a formal change control board (CCB) process. Any scope addition or change request after the initial sign-off must be approved by the CCB and result in a documented adjustment to the timeline or resources.

Why #: 3rd Why | Question: Why is the new-hire training inadequate? | Answer/Finding: The sales leadership and training department haven't updated the curriculum to reflect the new product suite and market dynamics introduced over the last 18 months.

Why #: 4th Why | Question: Why hasn't the training curriculum been updated? | Answer/Finding: The Sales VP views training as a cost center and continuously defers requests for necessary funds, resources, and dedicated staff time to overhaul the program.
